My name is Clare and I use the experience and skills developed throughout my career to deliver consultancy work for:

  • Teams looking to improve EoLC service or prepare for CQC

  • EoLC education

  • Lasting Power of Attorney

Proactive Advance Care Planning is a theme linking each service, and my work includes raising public awareness through Talks and Presentations

I believe passionately in proactive Advance Care Planning, campaigning to raise public awareness and professional understanding. I have seen first-hand and too often what can happen when vital conversations and plans are left too late and am on a mission to normalise talking about what matters most. You will find some great resource to support Advance Care Planning on the blog and,  video and through my publications

Launching  Conversations About Advance Care Planning in January 2022 is something I an exceptionally proud of. The weekly podcast explores Advance Care Planning, what it is, how to do it and why it matters through a series of conversations. Each episode has some fabulous resource and every guest focuses on a specific area of Advance Care Planning. The series shows that Advance Care Planning and it fits in with normal life planning. You can subscribe to podcast through all the usual providers.

My career and experience

I am a Registered General Nurse and have specialised in providing care for people at the end of life working in hospices, the community and acute hospitals.

I have worked in a national role for the Gold Standards Framework (GSF) to deliver the acute hospital GSF programme, supported delivery of the NHS Five Year Forward View plan working for local Clinical Commissioning Group and as a Director for a Practice Alliance.  I completed an MSc in Advanced and Specialist health care and have experience presenting at conferences.  My career has included clinical practice, education and quality assurance.  I continue to work as a Specialist Advisor for the Care Quality Commission.

Change

In 2020 I cared for a close relative at the end of life and experienced the challenges of working in a system that just doesn’t join up. It was a salutary lesson, making me question just how difficult it must be if you don’t have the luxury of background knowledge. It also provided a catalyst for my career change and you can read more here in an article published by the British Journal of Community Nursing.

Outside of work I enjoy travelling, attempting to grow vegetables, running and am a keen yoga practitioner. I am also a qualified fitness instructor and recently gained the PADI SCUBA diving certificate – a couple of ticks on my bucket list.
